Dental Insurance Accepted
Written Estimates and Co-Pays
After your treatment plan is developed, we will provide a written estimate of your expected financial obligation, including co-pays. Co-payments are due at the time of service.
Your Insurance Agreement
Our accounting team will prepare the necessary insurance forms. Your policy is an agreement between you and your insurance company, not between the insurer and our office. We cannot guarantee estimated coverage, but we will do our best to help you receive the maximum benefits available under your plan.
You remain responsible for the full balance if your insurance benefits provide less coverage than anticipated.
Pre-Treatment Estimates
If you would like to understand your expected coverage before treatment, we can submit a pre-treatment estimate. Your insurer will generally provide a detailed response within two to four weeks.
What to Bring
Please bring your dental benefits card to your appointment. Your dental plan may use a different carrier than your medical plan. If you were not issued a card, please bring:
- Insurance carrier name
- Claims mailing address and phone number
- Group or plan number
- Subscriber name and date of birth
- Subscriber ID number
Cosmetic Dentistry and Insurance
Cosmetic procedures are generally not covered by most dental insurance policies. Partial benefits may be available when teeth are broken or have previous fillings, bonding, or crowns.
Many insurance companies pay approximately 80% of the allowed cost for silver fillings on posterior teeth. If porcelain or tooth-colored material is selected, the insurer may base its payment on the cost of the less expensive silver filling. Coverage varies by plan, so your actual benefits should be confirmed with your insurance company.
